I tried using hot spot slides for my assessments, but those didn't really fit my needs so I started using slides with click boxes that reported to the quiz.
Here's how I have it set up. The first click box is set to go to the next slide if clicked correctly, but if the user fails then it just continues on in to the slide where there is a new text box and high light box instructing the user on what they did wrong and where they should click.
For some reason in the last few times I've run the preview it has just skipped over the pause in the first click box and gone straight to the second box. It pauses at the end of those boxes though. I've checked the settings and the boxes are set to pause until the user acts. For a while I only had the issue when just previewing, but not when previewing in the web browser, but now it's messing up even in the we browser.
Click boxes not pausing?Just tried your settings with the two click boxes and it works here, but I changed the order of the layers: the click box above the highlight box. I'm not sure at all, but give it a try?

I was able to work through the issue. I realized that the slides I was having issues with were directly after slides that had click boxes that needed double clicks to continue. I'm not sure if sometimes I was triple clicking by accident or if the system was reading it strangely, but the next slide's click boxes were being activated immediately.
I fixed the issue by moving the click boxes .2 seconds away from the start of the slide so that they now look like this:
Hopefully my own personal headache might help someone else out down the line.
